Required Paperwork Must Be Printed Single Sided. You can access the prepopulated paperwork from a link on your registration receipt OR in your Teamlinkt account using the instructions HERE. Alternatively, you can also use the links below to print off copies of the blank paperwork (will not be prepopulated with your athlete's information).

Players: Required

• AYF Participant Forms Package (Previously known as the Player Pack)
• Concussion Waiver
• Code of Conduct
• Report Card
• Birth Certificate & Copy (first-year athletes only)
• Physical dated in the current year OR Medical Release found in the Player Pack (only one is required)
